how to do a nomogram in r

hello
I am doing research about flap thickness. i did a multiple linear regression for the factors that may have an influence on flap thickness.

I want to do a nomogram to predict flap thickness using these factors but using r is all new to me (i imported an SPSS file to r) and I'm not a professional statistician.
I've already installed rms but I don't know to do the formula for a nomogram
please help me TT
