Data collection abilities in blogdown websites?

I currently have a personal website built using blogdown within R. However, I am also looking to build a database of voluntarily-provided information from visitors, for research purposes.

For instance, I work with aquaculture managers and we have discussed having a database of pond data (species stocked, oxygen levels, etc.) that they could submit. This data would be stored in a database that I could use for statistical and modeling projects to improve pond management practices.

Is it possible to create a custom page with blogdown that would allow for data entry boxes (say 10 boxes that managers could enter specific data in each) that they could then submit and I would have access to? I am assuming it would function something like a "comment" box, but the data would be sent to a database that I set up, rather than be seen publicly on the page.
