I'm not sure I understand your question, but do you mean the two first lines in the CSV file are the column names and you like to merge them together as column name? If so, that's not a normal structure for a csv file, but this could be solved programmatically.
Can you show us what the original csv file look like. Also, to get the best chances of response, you should try and create a reprex so people can more easily recreate the issue and help solving it.
